Supported Accommodation

Making the transition from hospital care back into the community is a big step, but St Matthews Healthcare can provide the supportive ‘step-down’ care needed for people who may have been undergoing treatment for chronic mental health problems, or who have been in locked rehabilitation, as they move towards independent living in the community.

Our three and four bedroom houses allow residents to build their confidence of managing their daily life, but with floating support at an agreed number of hours in each day to meet each individuals’ needs.

Extra Day Care  provision is provided at the St Matthews Unit to enable residents engage in a wide variety of leisure activities and social events.

Referrals

Referrals can be made by family members as well as by the NHS Health and Social Care Teams, Community Mental Health Teams and Clinical Nurse Specialist Teams for Younger People with Dementia.
