Step Down and Step Up Care Pathways
We aim is to break the cycle of re-admissions into acute inpatient care settings by providing effective care pathways based on individual requirements.

Our specialist mental health teams work together to provide intergrated, supportive care pathways including step downs from our hospitals.
This guide details the potential pathways available across St Matthews Healthcare. where service users can effectively step up or down, between hospital and community based care settings depending on their requirements.
Person-Centred Care Plans
All our service users have a person-centred care plan tailored to their specific needs, as well as a risk management strategy and outcome measures which are monitored and adjusted regularly in line with the individual’s changing needs. Risk assessments are carried out by trained assessors within the multi-disciplinary team or psychology department if they require a specialist input. Assessments are carried out in partnership with all appropriate clinical partners, including nursing, occupational therapy, medical and psychology teams, also involving other stakeholders including carers and external clinical partners.
Our Hospital Units Care For:
- Chronic enduring complex mental disorders with potential high risk to self and or others in Locked rehab units (male and female)
- Chronic enduring complex mental disorders with relatively low risk to self and or others in Open rehab (male and female)
- Acquired brain injury with or without mental illness but with behaviours that may challenge in Neuro behavioural unit (male)
- ASD, mild learning difficulty with comorbid mental illnesses supported through Transforming care agenda
- Acute mental illness episodes in Acute admission unit (male)

- Complex mental health needs stepping down from hospital requiring multi-disciplinary team support
- Individuals supported with S117 aftercare, Conditional Discharge, CTO, CTR, Guardianship Order or DOLs framework
- Dementia including Early onset dementia
- Behaviours that challenge
- Complex physical health needs
- Neurodegenerative disorders including Huntington’s Disease, and MS
- Complex care needs
- Neuro rehab level 2
