Preferred Service Provider
Since 2010 we have been a Preferred Service Provider for the East Midlands Locked Rehab Framework. And for the past 5 years, we have provided expert acute services to the region and beyond. We are a progressive, growing organisation that strives to be an outstanding care provider and employer of choice by putting people at the centre of everything we do.
Step Down and Step Up Care Pathways
We aim is to break the cycle of re-admissions into acute inpatient care settings by providing effective care pathways based on individual requirements.
Our specialist mental health teams work together to provide intergrated, supportive care pathways including step downs from our hospitals.
This guide details the potential pathways available across St Matthews Healthcare. where service users can effectively step up or down, between hospital and community based care settings depending on their requirements.
Person-Centred Care Plans
All our service users have a person-centred care plan tailored to their specific needs, as well as a risk management strategy and outcome measures which are monitored and adjusted regularly in line with the individual’s changing needs. Risk assessments are carried out by trained assessors within the multi-disciplinary team or psychology department if they require a specialist input. Assessments are carried out in partnership with all appropriate clinical partners, including nursing, occupational therapy, medical and psychology teams, also involving other stakeholders including carers and external clinical partners.
