Our specialist mental health  facilities in Coventry and Corby offer a full multi-disciplinary team (MDT) catering for the Transforming Care client group, Neuro rehabilitation level 2 and Step down for S117 aftercare provision.

Step Down Community-based MDT Pathways

We provide diverse, integrated hospital and community pathways where individuals are supported based on their needs to either step up (a Hospital Pathway) or step down (a Community Pathway).

This step down (or step up) community-based MDT model ensures that service users get the care they need at whatever stage of care journey they are at.

Why Use a Community-based MDT Rehab Pathway? 

  1. This  care model aims to break the cycle of re-admissions into acute inpatient care settings by providing effective care pathways based on individual requirements.  
  1. Often moving from a period in secure hospital accommodation requires a gradual supported process. Our step-down community-based MDT rehab care pathway provides people who have undergone a period of intense mental health treatment with ongoing therapies and support to help them get back on their feet in a community setting. 

Managing Risk in a Person-Centred Manner 

The step-down pathway enables service users to take positive risks that promote their independence and sense of wellbeing and achievement.  

We treat our service users with dignity and respect and try to empower them to make their own decisions and choices about the care they receive, while ensuring their safety and the safety of others. We do this by active discussion and feedback, providing accessible information, and by promoting the use of advocates to ensure the service user’s voice is being heard.  

Their bespoke care plan is tailored to their specific needs. This includes a risk management strategy and outcome measures which are monitored and adjusted regularly in line with the individual’s changing needs. Risk assessments are carried out by trained assessors within the multi-disciplinary team or psychology department if they require a specialist input.  

Our MDT teams can support individuals using various frameworks including:

  • Mental Health Act (37/41 conditional discharge, CTO, Guardianship order, S117 aftercare)
  • Mental Capacity Act
  • Care and Treatment Reviews as part of Transforming Care agenda.
