S117 Service Provision and Transforming Care
Our supportive step-down community-based MDT model provides service users the care they need to move forward on their healing and rehabilitation journey.
This step-down community-based MDT care model ensures that service users get the care they need with an aim to break the cycle of re-admission into acute inpatient care settings by providing ongoing MDT therapies and support to help them get back on their feet in a community setting.
At St Matthews Healthcare we offer a full multi-disciplinary team care provision for the Transforming Care client group, Neuro rehab level 2 and Step down for S117 aftercare provision. At Willowbrook House in Corby we specialise in the Transforming Care client group and Step down for S117 aftercare provision.

Willlowbrook House cares for the following client groups:
- Specialist nursing care for those with severe and enduring mental health problems combined with physical illness/disability
- Dementia care and young onset dementia care
- Behaviours that challenge
- Specialist care for those entitled to Section 117 aftercare
- Those who are on a Discharge to Assess Pathway
- People who require a Care Programme Approach (CPA), a Community Treatment Order (CTO), Conditional Discharge from MOJ or Guardianship Order under the Mental Health Act 1983
- Learning disability (Mild)
- Forensic history
The Multi-disciplinary Team at Willowbrook House includes:
- Visiting Consultant Psychiatrist
- Visiting GP
- Visiting Psychologist
- Visiting Occupational Therapist
- Assisted Psychologist
- Occupational Therapy Assistant
- Qualified Nurses
Residents are stratified in 3 Tier system as follows:
Tier 3 MDT Level 2 : High Dependency Unit Challenging Behaviour with MDT
- Residents require a higher level of support due to complex or challenging behaviours with mental disorder requiring MDT input.
- Dual diagnosis and complex co-morbid physical health
- Specialist care for those entitled to Section 117 aftercare
- People who require a Care Programme Approach (CPA), a Community Treatment Order (CTO), Conditional Discharge from MOJ or Guardianship Order under the Mental Health Act 1983
- Forensic history
- Mild learning disability
Tier 2 MDT level 1: Specialist Challenging Behaviour with RC
- Residents require a higher level of support due to complex or challenging behaviours with mental disorder requiring Consultant Psychiatrist monitoring
- Resident behaviour requiring 15 – 30 minutes monitoring
- Dual diagnosis and complex co-morbid physical health
- Specialist care for those entitled to Section 117 aftercare
Tier 1: Specialist Nursing Dependency
- Individuals with complex co-morbid physical health and mental health needs of a higher dependency
